Health literacy in practice requires the development of techniques that ensure that the patient can better access information, understand its content, know how to use this information, and make better health decisions. If the patient makes better health decisions, there are immediate reflexes in health outcomes. The aim is to develop an approach based on the commitment and creation of an atmosphere of trust that reduces uncertainty, anxiety, and embarrassment based on a process of assertive, clear, and positive communication (ACP model).

Cristina Vaz de Almeida has a Ph.D in Communication Sciences - Health Literacy She's a Researcher, Director of Post-graduate Studies on Health Literacy (ISPA), Lecturer, and Keynote speaker on Health Literacy. She has an extensive experience in the social, health, communication, and marketing domains. Has been planning and carrying out training in these areas in hospitals and other health organizations for over a decade. She has been publishing several books, chapters, and reports in the subject of Health Literacy, Health Communication, Social Marketing, Health Marketing; Social Inclusion. Susana Ramos holds a Master’s in Health Care Infection and Post-graduate in Patient Safety. Nurse Manager and Patient Safety Coordinator at the Centro Hospitalar Universitário de Lisboa Central. She is also member of the Quality and Safety Committee. Tutor in the International Course "Quality in Health and Patient Safety" at the Escola Nacional de Saúde Pública. Vice-President of Atua Saúde - Association for Patient Empowerment and Safety.
