Abstract

The growing demands of an aging population and rehabilitation needs strain global healthcare systems, necessitating efficient, personalized care delivery. Knowledge Management (KM) addresses these challenges by systematically capturing, sharing, and applying clinical expertise. This chapter explores KM's role in enhancing rehabilitation and aged care through digital health innovations, such as Artificial Intelligence (AI)-driven Clinical Decision Support Systems (CDSS), tele-assessment platforms, and wearable technologies. These tools improve patient outcomes by tackling data overload and fragmented care, with computational simulations and case examples like dementia tele-assessment demonstrating their efficacy.
